79-year-old female​

Medical history

Scleroderma, hepatitis, varicosities of the legs

Case details

  • Acute traumatic injury to the left leg due to recreational boat propeller with massive tissue disruption​

Intra-operative - Day 3

  • Initial Procedure: OR wound evaluation & debridement
  • Index Procedure (3 days later): Amputation of mangled limb and re-approximation of tissue​

Post Myriad application

  • Application of Myriad Matrix (10 x 20 cm (2x), 10 x 10 cm, 3-layer), trimmed to size, secured with suture to the posterior thigh and a separate application to the distal stump ​
  • Secondary dressing: Petroleum gauze contact layer and NPWT at 125 mmHg​

Day 5

  • Myriad well adhered, contact layer left intact ​
  • Myriad kept intact and re-hydrated​
  • No sign of infection or complication

Day 14

  • Myriad integrating with areas of residual graft noted and left intact​
  • Well-vascularized granulation tissue developing​
  • No sign of infection or complication​

Day 22

  • Significant development of organized granulation tissue ​
  • Depth of defect eradicated​
  • No sign of further infection or complications

Day 33

  • Further development of well-organized, vascular tissue
  • No sign of infection or complications
  • Plan for split-thickness skin graft (STSG) in 4 days

Day 37

  • Application of STSG performed

Day 40 (1 week after STSG)

  • Excellent take of STSG​
  • No complications

Week 8 (17 days after STSG)

  • Well-remodeled skin​
  • 100% take of STSG​
  • No sign of infection or complications​
